Help our clients manage their chronic and acute conditions in the context of their social determinants of health. Our clients have Medicaid insurance and live in the District of Columbia, primarily in Wards 7 and 8.

Essential Functions:
  • Engage clients at their residence and/or by video or telephone for structured assessments and care plans.
  • Collaborate with medical providers and admins to arrange medical care and community supports for clients.
  • Use the nursing process to improve all aspects of a client’s health, including social determinants.
  • Assess client’s current health status using standardized assessment tools and identifying all barriers to achieving their best health.
  • Define realistic measurable goals related to each problem.
  • Develop a nursing plan which is directed toward the optimal resolution of nursing diagnoses and prevention of future problems.
  • Evaluate and document a patient’s progress toward goals.
  • Collaborate with the appropriate healthcare professionals in implementing a comprehensive plan of care.
  • Provide an individualized plan of nursing care that assists the patient in maximizing participation in his/her care.
  • Collaborate with the physician to revise the nursing plan of care and/or goals as necessary.
  • Implement appropriate patient education programs to meet the individual’s healthcare needs.
  • Function independently to maintain work hours and productivity. You are a self-motivated individual who possesses critical thinking skills and excellent problem-solving capability.
  • Possess a private work area, a home computer with reliable high-speed internet to access a secure network via a virtual desktop for privacy and HIPAA compliance.
  • Provide distant patient education, triage issues, and provide nursing advice. This may include lab results, medication adjustments, medical advice, preventive care or appointment needs according to clinical best practice and physician instructions.
  • Document patient interactions within the EHR using the appropriate workflow.
  • Provide clinical documentation to PBM (Pharmacy Benefit Management) or Insurer to obtain authorization for medications, treatments, procedures, diagnostic testing and surgeries as needed.
  • Prevent urgent/emergency visits or hospitalization whenever possible by expediting appropriate appointments as clinically necessary and communicating with providers/physicians regarding patient disposition.
  • Supervise and coordinate with community support workers as directed who are assisting the same group of clients.
  • Coordinate with other Nurse Navigators as assigned to provide cross coverage for continuity of patient care during absences/leave and PTO.
  • Attend in person departmental meetings as needed with assigned department/specialty. At times staff and physicians may need to meet in person to discuss operations and departmental issues.
Qualifications:

Education Level: RN required, BSN preferred
Experience: 1 year as RN, preferably in community health
Licenses and Certifications: Nursing license in DC/District of Columbia preferred, or another state with ability to obtain in DC

Physical Demands: The navigator is primarily seated while performing the duties of the position. Occasional walking or standing is required. The hands are regularly used to write, type, and handle or feel small controls and objects. The navigator must frequently talk and hear.
