REPORT TO: Health Navigator Supervisor
Functions: Health Navigator (HN) is responsible for conducting health outreach activities in accordance with Urgent Wellness guidelines. The HN provides District of Columbia (DC) residents with screening, interventions, and coordination of care that are essential for achieving a one’s best health. Approximately 75% of the time, the HN will be engaged in activities out in the community and/or the client’s natural setting.
REQUIREMENTS:
Successful candidates must have completed at least two years of study at an institution of higher education or have at least two years of experience in a relevant human service field. Must have own transportation. Excellent communication and organizational skills, confidence with various software programs, and a passion for working with people and improving our community’s health. Experience working in a community-based and multi-cultural setting is preferred.
Most importantly, the successful candidate must be able to develop effective working relationships with the client population, be self-motivated and able to motivate others, be able to relate to individuals making significant life decisions.

PRINCIPAL ACTIVITIES:
- Manage a caseload of no more than 100 adults
- Engage clients at their residence and/or by video or telephone for structured assessments and care plans
- Collaborate with medical providers and admins to arrange medical care and community supports for clients
- Assist the client in the development of health-conscious life skills
- Provide and coordinate referrals to make sure that the client receives the requested assistance
- Conduct visits to the client home, hospital room and other natural settings
- Coordinate transportation for clients as needed
- Adhere to Urgent Wellness documentation requirements and submit billing notes within 24 hours of service
- Provide health crisis intervention to clients as necessary
- Meet assigned weekly/monthly productivity requirements (10 billable hours per week)
- Meet with supervisor weekly for supervision
- Complete all required trainings annually
- Perform duties as assigned

- The ability to safely operate a motor vehicle to transport oneself, clients, and program supplies as necessary.
- The physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear, and operate a computer.
- Move and lift light objects up to 30 pounds such as supplies, files, and equipment.
- Operate computer requiring continuous or repetitive hand/arm movements.
- The ability to remain in a sitting position for extended periods of time.

PAY POLICY AND FREQUENCY:
At Urgent Wellness, pay dates are twice per month, the 15th and 30/31st.
